Patients Rights

Information security and access to your medical record

We will never disclose patient identifiable information (other than directly related to your care) to third parties without your express written consent.

You have the right to access your own health record. Access encompasses:

• The right to obtain a copy of the record in permanent form, and
• The right to view a record without obtaining a copy.
• The right to have information explained where necessary, e.g. medical abbreviations.

Please write to the practice manager in the first instance. There may be a fee for this access. Minimum fee is £10.50.
